That sounds a lot like what happened to me when I had my wisdom teeth out, which accounts for 50% of my dentophobia (the dentist I had as a small child, which matches Hitomi's experience here, accounts for the other 50%). The oral surgeon got three of the teeth out okay, went to remove the fourth, and the Novocain wore off. I screamed and tried to tell him I could feel it, he didn't believe me and went back to what he was doing. Only when I screamed loud enough that the people in the waiting room were about to call the cops did I get another shot of Novocain. (When my brother had to have his wisdom teeth out, our family dentist was going to send him to the same oral surgeon, and my mother said, "Absolutely not." He got sent to one who actually put him under general anesthesia for the procedure.)
Hitomi definitely shouldn't be embarrassed about having a common fear. The silly boy was afraid it would detract from his "cool image." Little does he know that other people have fears of their own that they might consider just as embarrassing.
